Horsham Community Partnership Team
Horsham Partnerships for Older People works closely with the Horsham Community Partnership Team (CPT).
The CPT is a joint team including staff from West Sussex County Council, West Sussex Primary Care Trust, Department for Work and Pensions, and Horsham Citizens Advice Bureau.
The CPT includes:
- a health advisor (nurse)
- a social worker
- advisors from the Pension Service
- support workers from Horsham CAB
- volunteers
How can they help?
The CPT will work with an older person to advise or actively put in place a range of measures which will assist that person’s health, quality of life and ability to live independently. The CPT normally works with people who do not receive services from specialist health care teams or social care services.
They can work with older people to:
- check your general state of health and wellbeing
- help you identify what may benefit you
- enable you to make any changes
- let you know what resources may be useful to you
- help you to get services
- provide assistance to access practical and emotional support
For example, the CPT could:
- advise you about how to keep healthy – with your diet, giving up smoking or taking up exercise – and manage health issues
- help you maximise your income and assist with benefit or pension queries
- link you with agencies that can help you with practical tasks such as shopping, cleaning and gardening
- help you to access groups and activities – such as lunch clubs, hobby groups and so on
- help you keep your home safe, secure and warm
